We are always striving to improve our services, and your feedback can help! Whether you have a new idea for us to try or feedback on an experience at any of our locations, we want to hear about it. There are several ways for you to give us suggestions or let us know how we can serve you better.
- Complete a Patient Experience Form. If you are in any of our clinics, you can ask any staff member for the form. Or can click the button below to submit an electronic Patient Experience Form. Either way, the form will be reviewed and addressed by a senior member of our staff. The appropriate person on staff will respond to your feedback within 72 hours.
- You can join our Patient Voice committee. The committee was created to give patients a way to offer feedback on how Clinica operates and where we should focus our efforts. Both English and Spanish languages are accommodated. If you are interested, ask a staff member for more information.
- If we send you quarterly patient survey or a post-visit survey, please complete it. It only takes a few minutes.
